In response to COVID-19, all counseling sessions are currently conducted through my secure telehealth platforms.
Before your first appointment, please download and complete these three forms:
I have a secure file transfer service set up for you to safely upload and return your completed forms PRIOR to your first appointment. If you are unable to complete your forms before your scheduled session, please let me know so we can reschedule for after you’ve completed this necessary paperwork.
If you are unable to keep your appointment time, please notify me (702-336-7620) within 24 hours per my cancellation policy.
